Insurance & Payments

We are contracted with most major PPO plans, Medicare, Tricare, & several mental health plans. .

Copays, deductibles, & coinsurance will be collected at the time of visit. We accept all major credit cards through our secure payment merchant, or cash is accepted at in-person visits.

What to Expect:

  • Our office utilizes HIPAA & HITECH compliant software to communicate by SMS or email
  • Office forms are completed electronically using the 2-way communication platform
  • Automated appointment reminders enhance efficiency
  • New patients will need to provide a copy of their ID & insurance card
  • We look forward to serving you!

  • Random urine screening is required at the discretion of the provider which may include an additional charge if not covered by insurance.

Additional Fees: 
​

  • There is an additional fee for completion of forms for use outside of the patient's medical record.
